Özet (EN)
ABSTRACT Supporting the Alvarado Scoring System with radiologic images and crp, procalsitonin levels in patients with clinical diagnosis or suspect of acute apendicitis Acute appendicitis is the most common in all age groups cause of acute abdomen. Our aim to reduce the negative appendectomy and related complications in patients with suspected acute appendicitis. Alvarado Score is a scoring system described in 1986. The patient's symptoms, physical findings, and laboratory values consist of the examination. The purpose of this study, the value in the diagnosis of acute appendicitis, and the importance of Alvarado Score for additional diagnostic procedures to determine the effect. In the study between May 2012 and May 2013, Balcalı Hospital of Çukurova University Emergency Department and General Surgery outpatient clinic with suspected acute appendicitis were included 133 patients. Any restrictions imposed on the selection of patients in this study. Regardless of the treatment of patients with this study and that the planning was done by the physician. Twenty patients were discharged after observation. (%15). We have operated on 113 patients (85%). Ninety-four patients (83.2%) were abnormal in acute appendicitis. There weren't 19 patients with histopathologically appendicitis(16.8%). All patients with serum CRP and PCT values were investigated with routine laboratory investigation. Abdominal ultrasound in all patients on admission were taken. The differential diagnosis of clinically suspected patients requiring oral and IV contrast-enhanced CT were taken. Increase the Alvarado score in diagnosis by increasing the effectiveness of all these results were investigated. In our study, the sensitivity of Alvarado score of 85%, specificity of 46% found it to be. CRP and PCT in the case of Alvarado scoring with scores and increase the success rate found in diagnosis. However, we did not find a positive effect CT and USG on Alvarado scoring. We thought this was due to the different experiences of clinical radiologists to review. As a result, no additional diagnostic method alone is not able to put the diagnosis of acute appendicitis. However, we believe that additional diagnostic methods in cases with a low Alvarado score is a tool to clinicians to enable.
