Pd/pdl1 expression and its clinical relevance in patients with acute myeloid leukemia

Objective: AML; is a clonal disease characterized by the proliferation and accumulation of myeloid precursor cells in the bone marrow. Some studies have reported that high rate of PD1/PDL1 expression is poor prognostic in patients with acute myeloid leukemia. Blocking of the PD1/PDL1 mechanism increases the antitumor immune response by reactivating T cells. In this study, we aimed to evaluate the prognostic significance of PD1 and PDL1 expression in bone marrow samples of patients with AML. In our study, the relation of PD1/PDL1 expression with clinical and laboratory parameters was analyzed. Materials and Methods: In this study, 99 patients with AML referred to our Hematology Department of Çukurova University Hospital between March 2009 and September 2018 were included. Bone marrow aspiration and biopsies were examined and the diagnosis was made according to WHO criteria. Bone marrow biopsy material and peripheral blood samples taken during diagnosis were examined. For immunohistochemical staining, polysilicate slides were treated with PD-1 and PD-L1. In this application, Ventana brand BenchMark XT model automatic immunohistochemistry staining device was used. Results: In our study, % 89 of randomly selected AML patients had de novo AML and %11 had secondary AML. Of the patients, 49 were women and 50 were men. The mean age of all patients was 51±1.67.Patients who were not in remission with induction therapy were older than patients in remission (p<0.05) . PDL1 was seen in 9 patients and PD1 staining was seen in 4 patients. CD117 and CD34 antigen levels were found to be high in patients with PD1 positivity (p<0.05) . There was no relationship between PD1 / PDL1 level and survival (p>0.05) . Conclusion: Our data did not demonstrate a clinically relevant correlation with regard to PD1 / PDL1 expression in patients with AML. Further studies involving more patients with acute myeloid leukemia patients with prospective flow cytometry and immunohistochemistry for control point inhibitors are required.
