Abstract (EN)
Despite advances in medicine, acute pancreatitis is a major source of morbidity and mortality. Clinic is variable and etiology is known, but pathogenesis is not fully understood. Still today, there is not a specific biochemical parameter that can be used to determine the diagnosis and severity. The treatment is performed according to the severity of the disease. Today, severity in order to determine, the most using systems are Ranson, Glasgow and APACHE-II scoring systems.Ghrelin, is discovered in 1999, which known have many effects including anti-inflammatory properties. Obestatin which is the product of the same gene in the ghrelin, has an opposite effect, the studies showing that, but this is not yet clear. Aim of this study is, investigate the role of ghrelin and obestatin, on to diagnosis of acute pancreatitis and severity determination.Thirty patients who were the patient group and had diagnosis of AP and stayed in the service for treatment and 25 healty volunteer people who were the control group were added the study. The patients were divided into 2 groups as severe and light AP by using Atlanta criterias which was admitted in 1992. In the application period and after the diet blood examples were taken from patients and ghrelin and obestatin levels were analyzed. There were not a significant difference between light and severe AP patients? ghrelin and obestatin levels. When the control group is compared to the pancreatitis, it is observed that in both group fasting acyl ghrelin and des-acyl- ghrelin levels were increased. After oral taking it is seen that in control group levels decreased and in pancreatitis the level did not change. When we consider obestatin in the application period and after the diet fasting the levels of obestatin were high.Eventually, in diagnosis of AP; the level of ghrelin could not determined in the period of application. However after the staying in clinic the maintaining of high levels ghrelin can be supportive for diagnosis. High obestatin levels in application period can be more important for diagnosis. But there could not be determined a relation between these two hormones and the severity of AP.Key words: Acute pancreatitis, Obestatin, Ghrelin
