Retrospective evaluation of CMV reactivation in patients whoundergone allogeneic hematopoietic stem cell transplantation forhematologic malignancy

Abstract (EN)

Human Herpesvirus 5 better known as cytomegalovirus (CMV) is an important contributor to the morbidity and mortality associated with allogeneic hematopoietic stem cell transplantation. We aimed to detect CMV infection prevelance, risk factors, related factors for time to infection, treatment and associated adverse effects, relationship between CMV infection and relaps and survival after allogeneic stem cell transplantation. This study was retrospectively examined 110 hematologic malignant disease patients undergone allogeneic hematopoietic stem cell transplantation between 2011 June and 2019 November in Bursa Uludag Univercity Faculty of Medicine Internal Medicine Department Hematology Department Stem Cell Transplantation Unit. Among 110 patients, 4 of them was excluded due to missing data, statistical analysis was performed for 106 patients. CMV infection prevelance was 63.2% and there was 95 infection episode for 67 patients. Steroid usage is only factor that has significantly importance for CMV infection (p=0.01. Most common adverse effect was neutropenia (47.7%) and followed by thrombocytopenia (26.8%), increase in serum creatinine (16.4%) and anemia (8.9%). Median relaps free survival was 26 months and overall survival was 36 months. There is no statistically important relationship between CMV infection and relaps free survival (p=0.59). Although mortality was higher in CMV infection group than others, there is no statistically important relationship between CMV infection and overall survival (p=0.67). In conclusion, CMV infection is a frequent complication and important cause of morbidity and mortality for hematopoietic stem cell transplant recipients. Prospective, randomized controlled and with larger samples studies are needed for better outcomes for recipients. Keywords: Allogeneic HSCT, CMV infection, GVHD
