Evaluation of the effect of inferior oblique muscle weakening procedures

A number of different surgical procedures may be used in treatment of inferior oblique overaction (IOOA). The most commonly used surgical techniques are myectomy, disinsertion, recession and anterior transposition. Success rates of these surgical methods are reported in different studies. Surgeons prefer different methods based on success rates, simplicity and advantage-disadvantage properties. In this study, disinsertion was performed in 207 eyes of 128 patients with IOOA , and anteroposition was performed in 104 eyes of 55 patients with IOOA and DVD. Postoperative first month, third month and last examination data were evaluated. Considering the IOOA, success rate of 95,65 %, residual hyperfunction of 2,,41 % and hypofunction of 1,93% were found in disinsertion group. In anteroposition group success rate of 87,5%, residual hyperfunction of 10,57% and hypofunction of %1,92 were found. Adherence syndrome or anti-elevation syndrome was not observed in any of the patients. In conclusion, we consider that disinsertion provides satisfactory outcomes with less risk of complications in IOOA. Anteroposition is the appropriate treatment option in cases with DVD and IOOA. Both surgical methods are found as effective and safe procedures.
