Influence of surgical pleth index on determinating depth of anaesthesia

Özet (EN)

Background: The purpose of this study is to evaluate the effectiveness of the SPI (Surgical Pleth Index) utilization on patients undergoing breast surgery with general anesthesia. Methods: For this study, within six months after obtaining permission of Dokuz Eylul University School of Medicine Clinical and Laboratory Research Ethics Board, patiens whom got daily operation of breast surgery with general anesthesia, were included. Age, gender, height, weight, BMI, ASA classification of these patients were recorded. Data of routine monitoring hemodynamic parameters in the specified time intervals were recorded too. During maintenance of anesthesia, remifentanil infusion rate has been set based on clinical hemodynamic variables in Group K, followed by clinical findings but set based on SPI values (20-50) in Group C, used Sıugical Pleth Index (SPI). Operation time, recovery time, the total dose of the drug used and developed postoperative complications were recorded. Results: There was no significant difference between the groups in terms of the demographic datas, operation time and anesthesia time. There was significant difference between the groups in terms of the incidence of postoperative complications and total dose of remifentanyl (µg) used and recovery time. In Group C, remifentanil dose was found to consume significantly less and incidence of postoperative complications, recovery time and need for additional analgesic were found less. Conclusions: During daily operation of breast surgery with general anesthesia, using SPI as a guide of remifentanyl dosage reduces average consumption of remifentanil, recovery time, postoperative complications and need for additional analgesic. According this informations, during daily operation of breast surgery with general anesthesia, using SPI as a guide of remifentanyl dosage will be very useful and meaningful for quick recovery.
