Comparison of the effects of low flow desfluran and high flow desfluran on oxidative stress and dna damage in anesthesia applications

Introduction and Objective: Today, the increase in new and advanced anesthesia devices, especially considering the ecological pollution, has increased the interest in low flow anesthesia.At the same time, conditions such as cost reduction, closer follow-up of the patient, and reduction of intraoperative heat and moisture loss have increased the use of low flow anesthesia by anesthesiologists.In this study, we wanted to evaluate whether low-flow anesthesia is different from high-flow anesthesia in terms of oxidative stress and DNA damage. Materials and Methods: Sixty patients, aged between 18 and 70, with ASA I-II physical status, who underwent general anesthesia for three hours or more were included in this study.The patients were divided into two groups according to the intraoperative desflurane flow rate.Patients who underwent low-flow anesthesia (0.5lt/min) were named Group I, and patients who were administered high-flow anesthesia (4lt/min) were named Group II.The hemodynamic parameters, ETCO2 baseline values of the patients were recorded after induction, after intubation at 5, 10, 20 and every 15 minutes thereafter.TAS, TOS, MDA, Cystatin-C,8-OHdG and LPO parameters were studied for the evaluation of oxidative stress and DNA damage in the preoperative, intraoperative third hour and postoperative 24 hour. Results: 8-OhdG for DNA damage and MDA, LPO, Cystatin C, TAS, TOS were measured for oxidative stress from the preoperative, intraoperative and postoperative blood samples taken from the patients.Postoperative MDA value was found to be significantly higher in the high flow group than in the low flow group (p<0.05).When the groups were compared in terms of TAS, TOS, Cystatin-C and 8OhdG, no significant difference was found between the groups (p>0.05). Conclusion: In conclusion, in our study, we found that there was no biostatistically significant difference in DNA damage in the low-flow desflurane group compared to the high-flow desflurane group, and oxidative stress increased significantly in high-flow desflurane application.
