Abstract (EN)
Obesity is the accumulation of abnormal amounts of excess fat tissue in the body. According to the World Health Organization, the main cause of obesity is an imbalance between energy intake and spent. Obesity causes many health problems with its negative effects on organ systems and the psychosocial status. After bariatric surgery, which is one of the treatment options for obesity, patients suffer from sagging because of excessive weight loss and body contouring surgery (BCS) is needed to treat the sagging. In our study, we aimed to compare the changes in the body satisfaction of patients after bariatric surgery; evaluate the characteristics of patients who desire BCS due to skin sagging as a result of rapid weight loss and patients who do not desire BCS. We also aimed to compare differences between patients who have a Body Mass Index (BMI) of 40 kg/m2 or higher and the ones who have lower BMI then 40 kg/m2. We aimed to investigate the financial burden of BCSs on the healthcare system which are needed due to excess weight loss. In this study, anthropometric measurements and segmental body analysis were performed to 541 patients who applied to our clinic for bariatric surgery. Multidimensional Body Self Relations Questionnaire (MBSRQ) was applied to all patients. In the first postoperative year, massive skin sagging and BCS requests of patients were evaluated. We conclude that MBSRQ scores were affected by the changes in body measurements after bariatric surgery. During the first year of follow-up after bariatric surgery, most of the patients (90.62%) complained about sagging in at least one part of their body and 79.31% of the patients declared sagging and stated that they request BCS. It was observed that MBSRQ scores had an effect on BCS request. Sagging and BCS request rate were higher in patients with a BMI of 40 kg/m2 and above than patients who have lower BMI then 40 kg/m2. The estimated cost of BCS was also higher in this group. In conclusion, BCS costs should be considered when conducting studies related to the cost-effectiveness of the generally accepted BMI criterion for bariatric surgery.
