Comparison of video head impulse test findings before and after therapy maneuvers in individuals with benign paroxysmal positional vertigo

The purpose of this study is to compare the vHIT findings before and after therapy maneuvers in patients with BPPV, in order to determine the importance of vHIT in determining the efficacy of the therapy maneuvers. The study includes a total of 55 subjects (aged 19-65): 25 BPPV (Study Group) and 30 healthy subjects (Control Group). All patients underwent audiologic examination, VNG and vHIT. Modifiye Epley maneuvers were applied to BPPV individuals. The vHIT was repeated 1 week after finding recovery in positional nystagmus. In addition to observation of positional nystagmus, BDEA and VAS, which evaluated by the patient in terms of patient's dizziness and dizziness disability in terms of supporting improvement in BPPV, were applied before the therapy and 1 week after recovery in study group. There was no statistically significant difference between the study and control groups regarding the vHIT gains of all bilateral semicircular canals (SCC) (p> 0.05). In the study group, there was no statistically significant difference between the vHIT gains of all bilateral SCCs obtained before and after treatment (p> 0,05). In the control group, and before and after the treatment in the study group; there were no asymmetry or refixation saccades. Compared with pre-treatment, the post-treatment findings of the study group had significantly improved: the dizziness-related disability levels that are determined through BDEA, and the self-determined severity of dizziness had significantly decreased (p<0.001). In conclusion, the subjective evaluations that determine the severity of dizziness and vertigo-related disability in BPPV provide supportive information in determining the effectiveness of diagnosis and therapy maneuvers. It was observed that VOR was not affected in individuals with posterior SSK involvement who had BPPV for the first time. It was determined that vHIT is not a decisive test in assessing the efficacy of diagnostic and therapeutic maneuvers in BPPV.
