Abstract (EN)

Purpose: The aim of this study is to compare the histopathological findings which were obtained after biopsy with the early diagnosis occured with age, gender and clinical findings of the patients by laboratory tests and physical examination and renal biopsies done in our clinics and to determine our performance by revealing the percentage of the correlation between the early and histopathologic diagnoses.Instruments and Methods: 130 patients who had renal biopsies in the Nephrology Department of Celal Bayar University Faculty of Medicine during the years 2007-2011 and whose information is reachable in archives were used in the study. 5 of the patients were excluded from the study due to missing information. Indications of biopsy included NS, ANS, proteinuira, deterioration of proteinuira + renal functions tests, haematuria+proteinuira and haematuria+renal functions tests and haematuria. Renal diseases which were done biopsy were classified as glomerular diseases, tubulo interstitial nephropathies (TIN), vascular nephropathies (VN), end-stage renal disease (ESRD) and undiagnosed nephropayhies. Clinical and pathological findings of the patients were compared according to age and gender. 3 early diagnoses were determined for patients before biopsy.Findings: Glomerular diseases were most frequently (70,7 %) detected in patients which were done renal biopsy. While the most frequent histopathologic diagnosis was focal segmental glomerulosclerosis (FSGS), the second most frequent cause of glomerular disease was found to be amyloidosis. Among the patients who had the diagnosis of diabetic nephropaty, 27,3 % of them had no diagnosis of diabetes previously. % 4,5 of the patients which were done biopsy with various indications could not be diagnosed.There was a significant difference between kidney sizes in USG and echo grade. Sifnificant difference between both kidney sizes and the kidney ecos have been found regarding to kidney function disorders. In our clinic, we determine 3 early diagnoses before renal biopsy with regard to the clinical and laboratory findings of the patients. The percentage of the correlation between pathological diagnosis which became definite after the biopsy and early diagnoses were compared. We identified that the correlation between definite pathological diagnosis and our 1st, 2nd and 3rd early diagnoses was 42,9%, 24%, and 14,3%, respectively for all biopsies. In that case, the percentage of the correlation between any of our early diagnoses and the pathological diagnosis was 81,2 %.Results : The results of our study, in which we retrospectively examined the renal biopsies that were done in the last four years in the Department of Nephrology in CBU-Faculty of Medicine, were found compatible with the studies including long-term, multi-centered and large biopsy series in literature. When we have retrospectively evaluated our performance regarding the correlation percentages between our early diagnoses and the definite diagnoses after biopsy, we have found our performance successfull by identifying that our first 3 diagnoses highly (81,2 %) correlated with the right diagnosis . However, we think that as our biopsy series increase, this rate will also increase in the following years.Key words: Renal biopsy, Glomerulonephritis
