Evaluation of long-term outcomes of living kidney donors

ABSTRACT Evaluation of Long-Term Outcomes of Living Kidney Donors Akdeniz University Faculty of Medicine, Department of Internal Medicine, Specialization Thesis, Antalya, 2022. Introduction and Objective: In our study, it was aimed to evaluate the long-term results of live kidney donors and to compare the long-term results of SCD and ECD. Material and Method: January 2010 and December 2015, 590 people who had kidney donors at the Akdeniz University Faculty of Medicine Hospital Organ Transplant Center were included in the study. First of all, all donors were evaluated in terms of indicators related to their general demographic, socio-cultural and economic characteristics, survival, complications, and functional kidney capacity, and a table related to the general donor pool of our center was drawned. In the second stage, donors were divided into two groups as SCD and ECD groups and group comparisons were made in terms of similar parameters. Data were collected retrospectively and analyzes were made using the SPSS program. Results: The mean age of the donors was 43.9±11.3, 348 (59%) were female and 506 (85.8%) were married. The mean hospital stay was 5 days (4.8±1.4), and the post-transplant follow-up period was 97±21.3 months. Developed HT in 66 donors (11.2%), DM in 28 donors (4.7%), CAD in 19 donors (3.3%), and nephrolithiasis in 4 donors (0.7%). The mean eGFR values were found to be 105.22±13.74 mL/min preoperatively, 73.54±16 mL/min at discharge, and 78.98±16.75 mL/min at the final follow-up. There was 11 (1.9%) donor loss. Survival rates of the patients were determined as 99.8%, 99.8%, 99.5% and 97.8% respectively for 1, 2, 5 and 10 years. When evaluated according to donor types, there were person 440 (74.6%) with SCD and 150 (25.4%) with ECD. It was shown that there was no significant difference between the groups in terms of complications, survival and conditions that may affect functional kidney capacity, only the rate of DM development was higher in the ECD group (this condition was thought to be caused by more IFG and IGT), there was no development of ESRD, and there were significant increases in eGFR values in the Friedman analyzes of the donors of both groups. Conclusion: In our study, it was determined that living donor kidney transplantation is a healthy and safe procedure in suitable conditions for the donor, and they have similar risk factors with the healthy population. In addition, it has been shown that the appropriate, necessary of using ECD in conditions of meeting the growing need for organs and insufficient cadaveric organ donation and long-term results have also been successful.
