Evaluation of the different surgical methods in multiple gingival recessions' treatment

Today, each of the techniques used in the treatment of gingival recessions has disadvantages or advantages, and there is still no single treatment technique that will provide complete root surface closure. The aim of this study is to compare the application of coronally sliing flap and connective tissue graft alone with L-platelet rich fibrin (L-PRF) in the treatment of multiple gingival recessions. In the study, 30 different regions of 15 patients aged between 18-65 years with multiple and bilateral gingival recession (Recession type 1: gingival recession without loss of interproximal attachment) were included in 2 different groups. In patients included in Group I, connective tissue graft and coronally sliding flap were applied to the defect area. L-PRF was used in addition to the connective tissue graft with a flap sliding coronally to the defect area of the patients included in Group II. Clinical measurements (whole mouth plaque index (PI), pocket depth (PD), bleeding on probing (BoP), clinical attachment level (CAL), keratinized tissue width and thickness, buccal recession depth and width was repeated before the operation and at the 1st, 3rd and 6th months after the operation. At Post-operative 1.,2.,3.,4,5.,6.,7. and 14. days, VAS evaluation was performed. Oral Health Impact Scale (OHIP-14) questionnaire was conducted before and after the operation on the 7th and 14th days, and at the 1st and 3rd months. It was observed that there was no significant difference in terms of all clinical indices (PI, GI, PD, SK, CAL) evaluated before treatment in both treatment groups. When the post-treatment values were compared between the groups, it was observed that there was no statistically significant difference in terms of whole mouth clinical indices (p>0.05). When the pre- and post-treatment data of the withdrawal regions were compared between the groups, it was seen that there was no statistically significant difference between the groups in terms of CAL, keratinized tissue width and thickness, buccal recession depth and width, and PD values before treatment. While there was no statistically significant difference between Group I(CTG+CSF) and Group II (CTG+L-PRF+CSF) in the % increase in CRC obtained at the end of the 1st, 3rd, and 6th months after treatment (P>0.05); A significant increase was observed in the 1st, 3rd and 6th months compared to baseline in both groups (P <0.05). While a regular decrease was observed in the level of pain felt by days from day 1 to day 14 in both groups, between Group I (CTG + CSF) and Group II (CTG + L-PRF + CSF), 1st, 2nd, 3rd, 4th, No statistically significant difference was observed between the 5th, 6th, 7th, and 14th days (P>0.05).
