The effect of D vitamin on the cytology, HPV and metabolic patients with and without vitamin purchase history and determined cervical intraepitelial neoplasia and hpv (+) patients

Abstract (EN)

Objective: Human papilloma virus (HPV) is the primary cause of cervical cancer. In previous studies, the contribution of vitamin D to HPV infection and cervical intraepithelial neoplasia regression has been shown. Metabolic effects of vitamin D have also been demonstrated. The aim of our study was to evaluate the effects of HPV regression, glucose homeostasis parameters, lipid concentrations, and inflammation on biomarkers in patients with ASCUS, LGSIL and smear results of benign HPV positive cytology and with and without a history of vitamin D deficiency. Materials and Methods: At the beginning of the study, patients were classified according to age, presence of additional disease, body mass index, age of first coitus, smoking, parity, oral contraceptive use, chronic immunosuppression, and operation status. Patients with vitamin D use due to vitamin D deficiency were identified from the system and patients who received vitamin D3 supplement containing 300.000 İU oral solution for 6 months were identified. 110 patients were evaluated at the beginning of the study, but 10 patients were excluded from the study due to lack of control examination results. Fifty of the 100 high-risk hpv positive patients were divided into two groups as vitamin D supplementation and 50 without vitamin D supplementation. Changes in serum vitamin D concentrations of patients with and without vitamin D supplementation were determined from the system. The study included 29 HPV 16, 4 HPV 18, and 67 other high-risk HPV positive patients. Smears were in 4 LGSIL, 4 ASCUS and 92 smear normal results. Results: The patients were divided into two groups with and without a history of vitamin D use due to vitamin D deficiency. There were 50 patients in both groups. The mean age of the group without a history of vitamin D intake was 40.2 and the mean age of the group with a history of vitamin D use was 40.9. Disease rates, smoking, oral contraceptive use, operative status, body mass index, and mean age of first coitus were similar in both groups. The rate of parity was higher in the group without a history of vitamin D use. (p:0,016) No patients with chronic immunosuppression. HPV regression was not significant in the group with a history of vitamin D use. (p:0,804) The metabolic results of vitamin D significantly decreased insülin (p:<0,001) , total cholesterol (p:0,002) , triglyceride (p:0,019) and CRP (p:<0,001) values in the group with a history of vitamin D use. Discussion: As a result of our study, vitamin D intake does not cause HPV regression but it causes metabolic effects. It has positive effects on glucose homeostasis and lipid profile. In the light of previous studies, vitamin D may be a good alternative in the treatment of vaginal infection, but it cannot be a good alternative to HPV vaccine in terms of regressing cervical intraepithelial lesions. If we look at the metabolic effects, vitamin D can be recommended to patients in terms of their contribution to prevention or regression of diabetes, hyperlipidemia and atherosclerosis Key words:HPV, Pap Smear, D Vitamin
