Effects of Tacrolimus wound healing in experimental glaucoma filtering surgery

It is known that wound healing affects success of glaucoma filtering surgery in negative way. Antimetabolities and antifibrotic agents were being used to modulate wound healing. This study is done to evaluate the effects of tacrolimus glaucoma surgery on wound healing treatment.In this study, fifty five male New Zelland Albino rabbits randomly subdivided into 5 equal groups. Limbal based trabeculectomy applied to four groups. No surgery done to fifth groups and this group is taken into consideration as control group. Group I was consisted of rabbits in which trabeculectomy and postoperative drug application was not performed. %0,9 NaCl 4x1 eye drop was applied to Group II, inraoperative 0.4 mg/ml mitomycin-C was applied under scleral flap for 3 minutes to Group III, octreotid 3x10 µg eye drop was applied to Group IV and 0.3 mg/ml tacrolimus 4x1 eye drop was applied to group V for 14 days after surgical operation. Operated eyes of rabbits was enucleated at end of 14th day of operation, on histopathological sections which were taken from bleb sites, Masson-Trichrome and Hematoxylen-Eosin with fibroblasts and mononuclear cells were evaluated, additionally immunohistochemically TGF-ß and FGF-ß staining In treatment groups the number of fibroblasts were found less significantly compared to sham group (p<0.0001). Number of fibroblasts in Octreotide group were determined less than the mitomisin-C (p<0.001) and tacrolimus (p<0.001) groups. In terms of mononuclear cells number suppression, mitomycine-C, tacrolimus and octreotide groups are appointed more effective compared to sham group (p<0.0001). According to sham group, it was recognized that the immunohistochemical staining density of TGF and FGF are compressed (p<0.0001). But no difference has been found between the medicine groups (p>0.05).As a conclusion, when severe adverse effects of antimetabolities considered, topical tacrolimus and octreoitide are seems safer methods.
