Abstract (EN)

"Effects of erdosteine on hepatic ischemia-reperfusion injury" Bulent Ecevit University, Department of General Surgery, Zonguldak, 2014. Ischemia – reperfusion injury of the liver may be seen in various clinical conditions such as trauma, hemorrhagic shock, liver operations and transplantation. The mechanism of this injury hasn't yet been clearly explained. Many different pharmacologic agents and methods like hepatic ischemic preconditioning are used in order to prevent ischemia reperfusion injury (IRI). However IRI still can't be precisely prevented. There are reports that antioxidant and antiinflamatory erdostein has protective effects in different ischemia reperfusion injury models. In this study erdostein's effects on liver ischemia reperfusion injury (HIRI) are investigated. Materials and Methods; 3 groups of 8 rats in each are studied. Group 1 (Sham): only laparotomy is performed. Group 2 (HIRH): hepatic artery and portal vein which supply blood to rat's left and median liver lobes (segments 2-5) are clamped with non – traumatic vascular clamp for 60 minutes in order to create %70 hepatic ischemia. After 60 minutes, the clamp has been removed and reperfusion is allowed for 60 minutes. Group 3 (ERD): 100 mg/kg/day dose of erdostein given by gavage for 3 days before the ischemia and reperfusion injury with vascular clamping. Blood and liver tissue samples were taken for biochemical and pathologic evaluation. After the experimental protocol, all rats were sacrificed. Liver tissue and blood samples were taken. We studied ALT, AST, Uric Acid. We also measured Total Antioxidant Capacity (TAC) and Malondialdehyde (MDA) levels. In biochemical evaluation, we found that TAC levels were significantly lower in group 3 compared to group 1 and 2. ALT, AST, GGT, tissue MDA levels and in pathologic analyses; leukocyte infiltration, apoptosis, ischemic coagulative necrosis didnt differ between HIRH (Group 2) and Erdostein (Group 3) groups. In this study where we investigated Erdostein's role on liver's ischemia reperfusion injury; we found that serum transaminases, TAC, MDA and histopathologic parameters are partially improved with Erdostein in liver ischemia reperfusion damage but the difference wasn't statistically significant. This study shows that Erdostein has partial protective effects in HIRI, but further studies in larger group trials are needed. Keywords: Erdosteine, İschemia, Reperfusion, Liver, İnjury.
