Clinical, biochemical and histopathological investigation of the effect of quercetin and PRP applications on wound healing in rabbits with experimental deep corneal wounds

In this thesis study; The aim of this study was to investigate the effects of Quercetin and E-PRP application on wound healing in experimental corneal wounds in rabbits, clinically, biochemically and histopathologically. The animal material of the study consists of 46 male New Zealand Albino rabbits with a weight of 2.5-3.5 kg. Rabbits were randomly divided into 5 groups. These groups are; group 1 (Control -Tobrased 0.3%- group, (n=6)), group 2 (E-PRP group, (n=10)), group 3 (1 mg/kg Quercetin group, (n=10)), group 4 (5 mg/kg Quercetin group, (n=10)) and group 5 (1 mg/kg Quercetin + E-PRP, (n=10)). Tobrased drops were applied to all groups in the study 4 times a day throughout the study. Each rabbit's own PRP was applied 4 times a day to the groups in which PRP was applied. In the groups in which quercetin was used, quercetin was given orally to each rabbit for 10 days at the doses specified in the groups. Half of the rabbits in the groups were sacrificed on the 11th day of the study, and the remaining half were sacrificed on the 42nd day, and samples were taken for histopathological examination. Purulent discharge was found in the PRP, PRP-Q and Q1 groups in the macroscopic examinations performed during the study period, and these findings were considered statistically significant. No purulent discharge was observed in other groups and it was not statistically significant (P>0.05). Significant differences were observed between all groups in terms of fluorescein staining (p<0.05). As of the 28th day of the study, the lesioned area formed in the cornea was not stained with fluorescein dye in all groups. When the differences between the groups were examined, the PRP-Q1 group showed faster recovery on the 28th day than the other groups. In the comparison between the groups in terms of TNF-α, it was observed that TNF-α was significantly higher on the 11th day in the PRP and PRP-Q1 groups compared to the K and Q5 groups (p <0.001). Although the Q1 group was numerically higher than the K and Q5 groups, it did not show a significant difference (p> 0.05). In our study, significant differences were found in PDGF-BB value measurements according to time. When the groups are evaluated within themselves; There was a decrease in PDGF-BB value over time in the PRP-Q1 group. As Vesauloma (1997) mentioned, in the Q5 group, PDGF-BB value was high on the 11th day when the inflammatory cells were high, and PDGF-BB values came to normal levels with the decrease of inflammatory cells over time. In this study, the MMP-9 level, which was high on the first day of the study in the PRP and PRP-Q1 groups, decreased over time, and this decrease was statistically significant (p<0.05). In the Q5 group, there was a significant increase on the 11th day of the study and a decrease was observed on the 42nd day (p<0.05). In this comprehensive and long-term study, in parallel with our hypothesis, it was determined that the application of strong antioxidant Quercetin at a dose of 5 mg/Kg for 10 days had a positive effect on corneal wound healing. It has been concluded that these findings will shed light on future studies and there is a need for detailed studies with longer-term applications of Quercetin.
