Comparison of metabolic parameters in pre-treatment and post-treatment in F-18 FDG PET/CT in patients with diffuse large B cell lymphoma and prognostic significance

Abstract (EN)

F-18 FDG PET/CT semiquantitative parameters [(SUVmax, diameter of the largest lesion (MTD), metabolic tumor volume (MTV), bulky lesion glycolysis (BLG), total MTV (TMTV), total lesion glycolysis (TLG), tumor SUVmax/liver SUVmax (TLr) and percentage change in parameters (ΔSUVmax, ΔMTV, ΔTLG)] were compared with Ann Arbor Stage, Deauville score pre-treatment (0), inter-treatment (i) and end-of-treatment (eot) values in 80 Diffuse Large B Cell Lymphoma (DLBCL) cases and aimed to investigate their relationship with prognosis in this study.Significant differences were found in the mean values of TMTV PET0, SUVmax PETi, Deauville PETi, TLr PETi, ΔSUVmax PETi, ΔTLG PETi between survivors and deceased.A significant difference was found between the mean values of MTD PET0, MTV PET0, BLG PET0, SUVmax PETi, SUVmax PETeot, TMTV PET0, TMTV PETi, TMTV PETeot, TLG PET0, TLG PETi, TLG PETeot, Deauville PETi, Deauville PETeot, TLr PETi, TLr PETeot, ΔSUVmax PETi, ΔSUVmax PETeot, ΔTMTV PETi, ΔTMTV PETeot, ΔTLG PETi, ΔTLG PETeot between survivors and those who died or had relapse.In ROC analysis for survivors and deceased, the cut-off point was calculated as 195 for TMTV PET0 and 101 for ΔSUVmax PETi.In the ROC analysis performed on survivors and those with relapse, the cut-off point was calculated as 5.675 for MTD PET0; 39.5 for MTV PET0; 530.900 for BLG PET0; 1821 for TLG PET0, 172.5 for TMTV PET0; and 99.94 for ΔTLG PETi.A significant difference was found in disease-free survival time between relapsed and survivors in those above and below the values obtained according to the cut-off point in the ROC curves of MTD PET0, MTV PET0, BLG PET0, TLG PET0, TMTV PET0, ΔTLG PETi.The results of our study showed that semiquantitative PET/CT parameters may be helpful in predicting prognosis in DLBCL.
