Research of the subtypes cytogenetically in diffuse large b cell lymphomas and their prognostic characteristics

Özet (EN)

Aim: Diffuse Large B cell lymphomas (DLBCL) is the mostly encountered Non-Hodgkin lymphoma type with the adults. It shows heterogeneity clinically, morphologically, immuno-phenotypically and cytogenetically. Because of this reason, they have different clinical course and survival. The aim of this study is; to put forward the characteristics of DLBCL showing this heterogeneity and also to study the relationship of the EBV RNA presence and C-myc translocation with the clinicopathological parameters and to reveal the influence of this rela-ionship on prognosis and survival. Methods: In our study; 50 cases with the DLBCL were investigated. Bcl-2, Bcl-6, CD30 and Ki-67 expressions are specified by means of the immunhistochemical methods. EBER (EBV RNA) presence is studied with the Cromogenic Insitu Hybridization method (CISH) and the C-myc rearrangement presence is studied with the Florescent Insitu Hybridization (FISH) method. Nonetheless the influence of these values on prognosis and survival were investi-gated. Results: Whenever the relationships between the ages, sex, LDH levels, IPI score results and the B symptom presence of the cases and their survivals compared; it was meaningful that the rate of presence of the B symptom with the males was higher statistically (p=0.005). In addition to this, EBV RNA rearrangement is determined with 13 cases (26%) and C-myc rear-rangement is determined with 6 cases (%12). Bcl-2 expression is evaluated in 26 cases (52%) and Bcl-6 expression in 27 cases (54%) Ki-67 proliferation index is evaluated as <50% in 17 cases (34%) and 50% and above in 33 cases (66%). No meaningful result could be found sta-tistically between Bcl-2, Bcl-6 expression, Ki-67 proliferation index, c-myc rearrangement and EBV RNA presence and the survival. Conclusion: DLBCL?s have different clinical course, different response in treatment and survival because of the fact that they form a heterogeneous group histomorphologically and genetically. These genetic differences should be revealed in order to specify the effective treatments and to extend the survival. For this purpose; there is need for the widespread studies having regular clinical follow-up. Key words: Diffuse Large B cell Lymphoma, Double Hit Lymphoma, C-myc rearrangement, EBV RNA
