ABSTRACT COMPARING THE SERUM OSTEOPONTIN LEVELS OF THE PATIENS WITH DIPPER AND NON-DIPPER HYPETENSION Hypertension is an important cause of mortality and morbidity with its preventable characteristics affecting millions of people all over the world. The questions on hypertension has led to the birth of dipper and non-dipper blood pressure concepts. The purpose of our study is to evaluate the Osteopontin (OPN) levels in our hypertension patients who were separated as dipper and non-dipper groups whose ambulatory blood pressures were observed by us; and we also aimed to determine the difference between the groups. The hypertension patients who applied to Fırat University Hospital, Cardiology Department Polyclinic between the dates January 1, 2014 and September 15 2014 were evaluated prospectively in the study. The patients who met the criteria for being included in the study were observed for ambulatory blood pressure. With the measurements made with AKBI, the blood pressure that was measured at night was diagnosed as dipper hypertension because it decreased 10% or more when compared with the values measured during the day; and as non-dipper hypertension because there was less than 10% decrease. In terms of the homogenization of the study, 40 patients from each group were planned to be evaluated. With the evaluation of patients in sufficient number from each group, the study was completed. Echocardiographic examinations were performed in the patients who were included in the study, and the serum OPN level was measured with the ELIZA Test with the blood samples taken from the patients. A meaningful difference between the serum osteopontin levels of the patients with dipper and non-dipper hypertension who were included in the study was detected (NDHT: 215.5±114.4, DHT: 152.1±92.7 p= 0,008). No statistically meaningful difference between the male and female genders in terms of the OPN levels was observed (K: 171.59±105.32, E: 220.44±111.45 p=0,80). In the NDHT Group, a statistically meaningful difference was detected in the left atrium diameter/level, in the left ventricle mass according to the DHT Group (IVS; NDHT: 11.6±1.2, DHT: 10.3±1.5 p<0.001). The Total Cholesterol Level was found as being statistically higher at significant level in the Non-dipper Hypertension Group wen compared with the DHT Group (NDHT: 209.1±28.2, DHT: 189.7±33.2 p=0.006). In conclusion, according to the data we have obtained, a statistically meaningful difference has been detected between the OPN level of the NDHT patient group, and the OPN level of the DHT Patient Group. No meaningful difference was observed between the genders. The Heart Mass Increase and the Blood Lipid Level in the NDHT Patient Group were observed as being higher when compared with the DHT Patient Group.
