Abstract (EN)
Chronic wounds are an essential health problem that affects a large number of patients and imposes a severe financial burden on both the patient and the health system, and this burden can be ignored. Wound healing is the restoration of the cellular, biochemical and systemic processes that begin with trauma to standard with the formation of new tissue. Metabolic diseases such as diabetes mellitus negatively affect cellular activities in the normal wound-healing process and cause chronic wounds that do not heal/heal poorly. Most chronic wounds require complementary, integrative, and conventional treatments in modern medicine. Many chemical, physical and biological agents that affect wound healing, especially topically, have been described. Topically applied methods include the first state of medicine in many cultures, phytotherapeutic treatment based on the use of plants (herbalism) for healing purposes, and biological called maggot debridement therapy (MDT) or larval debridement therapy (LDT), which includes the application of live fly larvae to wounds based on the use of animal natural resources for healing purposes. Therapy is effective methods that have been investigated for use in wound healing pathology. In this study, L. sericata fly larvae for wound treatment were first cared for and sterilized. The concentration and duration of the sterilant are standardized to obtain maximum viable individuals.In the current study, commercial wound healing ointment Furacin ®, secretion obtained from L. sericata fly sterile larvae, and cream prepared from endemic Achillea sintenisii plant extract from phytotherapeutic Achillea species were used in excisional wounds of 15-20 mm diameter in Wistar albino rats made diabetic with streptozotocin (STZ) 60 mg/kg. It was aimed to histopathologically examine the effects and rates of wound healing of the combined use of ointment prepared from maggot secretion and Achillea sintenisii plant extract. For this purpose, 24 Wistar breed male rats were randomly divided into four groups of six. The created working groups are DF, DM, DA, DM+DA. After shaving and sterilization under general anesthesia in the groups, excisional wounds ix were made in the cranial, median and caudal regions of the back in separate rats within the group, including all layers, and the treatment was applied topically to these wounds at the same time daily. 12. At the end of the day, samples taken from the sacrificed rats were sent to the laboratory for histopathological evaluation. As a result of the presented study, it was determined that L. sericata larvae secretions as an animal treatment were more effective in healing wounds caused by diabetes than the pomade prepared from A. sintenisii plant extract and the commercial wound healing cream Furacin® applied as an herbal treatment. Macroscopically, the fastest and most effective wound healing activity was detected in Group Diabetic Maggot + Diabetic Achillea sintenisii (DM+DA), where animal and herbal biotherapeutic agents were applied combined, on the 0th, 4th, 8th and 12th-day wound follow-up. The highest histopathological scores were recorded in Group Diabetic Maggot + Diabetic Achillea sintenisii (DM+DA) on the 12th day (p<0.05). By identifying the larval (maggot) secretion and wound-healing active ingredients of the endemic plant Achillea sintenisii, different formulations can be developed and low-cost, easy-to-use, fast and effective commercial natural products that can be used in veterinary and medical medicine can be produced and offered for chronic wound care.
