Abstract (EN)

Objective: Magnetic resonance imaging of the knee joint pathology (MRI) to show the method has also aimed to investigate the effect of age. Materials - Methods: This prospective comparative cohort study was initiated after the local ethics committee approval (Number: 576/507, decision no: 15-2.1 / 10, date: 24/04/2015). Between May 2015 and May 2016, planned arthroscopic knee joint; at least 1.5 Tesla taken with the device in the magnetic field strength, at least three plans, proton-weighted or proton + T2, patients who imaged MRI in a sensitive to at least one plan fat-suppression fluid sequences were included in this study. The patients were under the age of 40 years (group 1) or above (including 40) (group 2) were classified as. Both groups were included 30 patients. Anterior cruciate ligament (ACL) and meniscus cartilage lesions on MRI sensitivity, specificity and diagnostic accuracy rate were investigated. Accuracy rates were compared between groups of MRI compatibility with arthroscopic findings. arthroscopic findings on MRI scans were evaluated by an experienced radiologist blinded. Statistical analysis was performed using SPSS 22.0 and MedCalc 14 programs. Fisher's Exact test, Cohen's Kappa test, Monte Carlo simulation method, Fisher's Z Transformation method and Student's t-test was used. Results at 95% confidence interval, significant when p <0.05. Results: In both age groups were 100% sensitive MRI imaging of anterior cruciate ligament rupture. Specificity and diagnostic accuracy rate of 100% in group 1, group 2 was 96.2% and 96.7%, respectively. Kappa coefficient group 1 1 (p <0.001), group 2 0,870 (p <0.001), which is found in a very good level of alignment between MRI and arthroscopic findings in both age groups. The sensitivity of MRI imaging of the medial meniscus lesions, specificity and diagnostic accuracy rate in group 1, respectively 70%, 95%, 86.7%; 81.8% in group 2, 87.5%, is 83,3 %. MRI and arthroscopy were good level compatible (Kappa coefficient of 0.684 and 0.619 respectively, p <0.001). Torn by type of diagnostic accuracy rate of 86.7% in group 1, group 2, 80% was 86.7% and 76.7%, respectively, if on the settlement of the tear. the display of the lateral meniscus lesions, MRI has low sensitivity in both age groups (Group 1: 64.3%; group 2: 12.5%); 100% specificity, 83.3% in group 1 diagnostic accuracy rate was 76.7% in group 2. While a good level of alignment between the findings in group 1, group 2 is negligible (kappa coefficients of 0.658 (p <0.001) and 0.173 (p = 0.092)). Torn type of diagnostic accuracy rate in two groups according to age in the place of residence is based on 80% accuracy rate in group 1, 80%, and 76.7% in group 2. MRI sensitivity in the cartilage lesions, specificity and diagnostic accuracy rate of 57.1% in group 1, respectively, 78.3%, 73.3%; In group 2 / 77.3, 50% and 70%. Both age groups are also weak level of harmony between MRI and arthroscopic findings (kappa coefficients of 0.322 (p = 0.153) and 0.262 (p = 0.195)). Results: In this study, MRI was found in all age groups in recognition of ACL rupture has a high accuracy rate. In recognition of meniscal lesions in determining the type and location of the tear accuracy rate of MRI it was found to decrease with age. The show cartilage lesions routine MRI sequences are known to be inadequate. In this study, demonstrating the cartilage lesions in both groups, but was lower in patients over the age of 40 diagnostic accuracy of MRI were significantly lower than in other pathologies. In recognition of the age and pathology in MRI of the knee joint it was concluded that there is a significant effect.
