Abstract (EN)

INTRODUCTION: Chronic myeloid leukemia (CML) is a stem cell disease with abnormal clonal proliferation of progenitor cells, constitute 15 % of adult leukemia. It has known to be CML is a fatal disease but since 1998, with the use of a tyrosine kinase inhibitor (imatinib mesylate) cml became a long-term evaluated chronic phase disease. Because of long expected survival in cml patients and needs to be treated for life; the cost of treatment should be considered and In resistant cases, effective treatment changes must be done at the right time. AIM/HYPOTHESIS: This study aims to investigate that effective management of tki treatment and overall survival in cml patients. Additional aim is to discuss the results with current literature. MATERIAL AND METHOD: This study included 88 cml patient who are treating with tyrosine kinase inhibitors . the clinical, demographıc and treatment response properties of these patients were evaluated. RESULTS: The mean follow- up period were 5.5 years (67 months) and overall survival (OS ) were 83% . Overall survival rate is close to IRIS (the study which evaluated imatinib response rates) but According to 2013 data of ELN (European leukemianet)it is lagging behind. İn resistant patient, second and third generation tyrosine kinase inhibitors ( dasatinib , nilotinib , ponatinib ) treatment response rates were similar the current literature. Some of newly diagnosed CML patients treatment was initiated with the other generic formulation of imatinib, these patient treatment response rates are similiar to first imatinib formulation. CONCLUSION: Considering the cost of treatment, imatinib generic drug formulations may be preferred as first imatinib Formula without any response loss. In CML patients long- term survival and disease-free survival can be obtained as a current literature data , with effective monitoring cytogenetic- molecular response and treatment changing at the rigt time with considering the patient co-morbidity KEY WORDS: Chronic myeloid leukemia, imatinib mesilat, dasatinib, nilotinib, ponatinib.
