Results of treatment for adult distal humerus 1/3 fracture, which DO NOT have any extension towards joint

No consensus has still provided for the treatment of extra-articular adult humerus 1/3 distal diaphysis fractures. We have aimed to evaluate the functional and radiological results of surgical treatment for the indicated fracture areas in our clinic. In this retrospective study, 31 patients with adequate follow-up, who were operated on extra-articular adult humerus 1/3 distal diaphysis fracture between the dates of January 2010 and October 2014, were included in the study. There were 22 men and 9 women cases. The mean age was 35,25±1,57 (19-72) years old. 8 of the fractures were open fracture, 27 were closed fractures. According to Gustilo-Anderson claasification, 3 of the open fractures were Type 1, 2 of them were Type 2 and 3 of them were Type 3. The mean follow-up period of the cases was 41,22±1,66 (24-80) months. Type of trauma; traffic accidents in 20, simple fall in 4, fall from height in 3, sports injury in 2, firearm injuries in 1 and being trapped in a wreckage in 1. The mean duration for being taken into operation was 5,06±4,77 (1-23) days. 20 of the cases were applied single plaque (Group I) and 11 were applied osteosynthesis with double plaque (Group II). Group I and II were statistically similar as per their sex, area, age, and according to AO and Gustilo-Anderson classification, as per their fracture type, duration of being taken into operation and follow-up period. In Group I; mean duration of operation was 97±30,7 (40-170) minutes, blood transfusion was 157,5±245 (0-700) ml, duration of bone union was 5,30±3,81 (2-18) months, in the seven of them (%35) bone union delay and in the three of them (%15) nonunion were experienced. Also in Group I; The average Mayo Score is 86,5±12,3 (65-100), ROM is 117,50˚±15,4º (90º-140º), extension loss is 7,25˚±8,95º (0º-30º) and the difference in transfer angle is 1,5˚±2,3º (0-7º). In Group II; mean duration of operation 153±46,6 (75-235) minutes, blood transfusion is 63±140 (0-350) ml, duration of bone union 3,36±0,67 (3-5) months, and one of them is (%9) experienced bone union delay. Also in Group II; mean Mayo Score was 84,54±10,3 (65-100), ROM was 116,82˚±12,8º (90º-130º), the extension loss was 8,18˚±8,14º (0-20º) and the difference in transfer angle was 1,09˚±1,7º (0-5º) Among the groups; no statistical significant difference has been determined between Mayo Score, Cassebaum grading, ROM, loss of extension and transfer angle. A statistically significant difference was found in favor of Group II in terms of operation time, duration of bone union, bone union problem between the groups. Related to the treatment of extra-articular adult humerus 1/3 distal body fractures; we recommend osteosynthesis with double plate because of shorter bone union duration and less complication rate.
