Retrospective analysis of the effect of serum 25 (Oh) vitamin D levels on insulin resistance and nutritional status in children and adolescent with exogenous obesity,

This study was conducted to investigate the effect of serum 25 (OH) vitamin D level and insulin resistance on nutritional status in children and adolescents with exogenous obesity. The study was conducted retrospectively with a questionnaire form prepared to collect the necessary information in the files of 209 individuals aged 6-18 years who were admitted to Ankara Başkent Hospital Pediatrics Outpatient Clinic between January 2015 and December 2021 and diagnosed with obesity. In the questionnaire form, the participants' gender, age, body weight, height and blood parameters were questioned. The mean age of all individuals was 11.9±3.23 years. The mean body mass index (BMI) of the individuals participating in the study was determined as 25.7±4.01 kg/m2 according to age. The rate of individuals with deficient serum 25 (OH) vitamin D levels was 26.4%, 41.1% with insufficient levels and 32.5% with normal levels. It was determined that 53.2% of mildly obese and 26.5% of obese individuals had normal serum 25 (OH) vitamin D levels. Among the obese and slightly obese children and adolescents included in the study, 37.2% of those without insulin resistance and 27.1% of those with insulin resistance had normal serum 25 (OH) vitamin D levels. The mean BMI of individuals without insulin resistance was 24.7±3.95 kg/m2. The same values were determined as 26.8±3.83 kg/m2 in individuals with insulin resistance. The BMI of individuals with insulin resistance was significantly higher than that of individuals without insulin resistance. In addition, a positive relationship was found between HOMA-IR and BMI. According to this relationship, an increase of 1 unit in HOMA-IR value caused an increase of 1.040 units in BMI value (p<0.05). In conclusion; childhood obesity is a risk factor for insulin resistance. Insulin resistance, like obesity, is a metabolic disorder that can be prevented and the risk can be reduced with dietary changes and physical activity. The finding of a significant relationship between vitamin D levels in obese children and adolescents with insulin resistance suggests that vitamin D therapy may be effective in the pathogenesis of these diseases.
