Mean follow results of open tibial diafiz fractures which treated with external fixation

From ancient times, tibia open fractures are one of the most importantproblems of orthopaedics and traumatology. There is no discussion about thefactor of soft tissue on the other hand there are some avaliable discusionsabout the best fixation?s method.Increasing stabilization with technology external fixations one moreusefull and less invazive. Pieced or loss bone in contamine wounded defendingendosteal and periosteal circulation performed a good option of treatment.Range of this study date between 1999 December and 2008 October,visited Turgut Özal Medical Center because of open tibia fracture who treat 73patient with external fixation. This study included adaquate following fourtyseven patient?s 48 open tibia fracture. Avarage following duration was 58,3month (distrubition 13?112 month). Avarage duration of patient?s stayinghospital was 23,3 days (distrubition 6?90 days). Eleven (% 23,6) patients werewomen, thirty six (% 76,4) patients were men. Our open fractured patients areclassified according to Gustilo Anderson (26). According to this 4 fractures weretype I (% 8,3), 18 fractures type II (% 37,5), 11 fractures type III A (% 22,9), 8fractures type III B (% 16,7) , 7 fractures type III C (% 14,6).Avarage duration of external fixation take off from patients aredetermined about 24,06±12,44 weeks (distrubition 12?54 weeks). Union is9 8obtained all our patients. Determined the union avarage duration is 27,27±13,19weeks (distrubition 12?56 weeks).Pin tract infection was most comman problem. Pin tract infectionrecognised 12 (% 25,53 ) patients. Deep skin infection was developed our 5 (%10,63) patient?s fracture area. Osteomyelit was developed in 4 (% 8,3) patients.Functional results are made according to Karlstrom-Olerud functionalevaluation criterion. Result of this 6 (% 12,5) patient is exellent, 16 (% 33,3)patient is good, 8 (% 16,7) patient is satisfied, 10 (20,8) patient is medium and 8(% 16,7) patient is evaluated as bad.As a conclusion, it seemed meaningful difference between open tibiafracture types of union duration as statistic (p=0,001). Functional scores foundmore higher in open tibia fracture type I and type II, than type III. There wasmeaningful differences between open tibia fracture types point of viewfunctional scores as statistic (p=0,0001).External fixing methods are reliable method for using emergency andpermanent treatments in open tibia fractures because of low infection, range ofhigh union, low malunion.Instead of determined which fixation method we use in fracture fixation, itrequires standart groups according to amount pieced fractured and type offractures. We need studies of making compare between different treatmentmethods for apply this constitute groups.
