Controlling haemodynamic response to extubation; comparison of the effects of lidocaine,esmolol and deksmedetomidin

Abstract (EN)

Tracheal extubation causes to increase the heart rate and arterial blood pressure and also the level of plasma katekalomin.This can cause some extracurricular results in some patient grups. In this study,we compared the effects of lidocaine,esmolol and deksmedetomin on haemodynamic changes during tracheal extubation.Material and Methods:Patients;Group I (Deksmedetomidin, n=20), Group II (Lidocaine, n=20), Group III(Esmolol, n=20) ve Grup IV (Placebo, n=20) as if;divided into 4 grups.During the induction, 1?2 ?g/kg fentanyl, 2 mg/kg propofol and 1 mg/kg vekuronyum is used. Endotracheal intubasyon existed after 90 seconds.Intubation continues with the consantration of %2 end-tidal in solution %50/50 O2/Air.5 minutes before the extubation , 1 ?/kg deksmedetomidin in 10 ml to Group1, 2 mg/kg lidocaine to Group 2, 2 mg/kg esmolol to Group 3, 10 ml SF to Group 4 is given by infusion pomps.The patients are extubated by aspirating the orofaringeal secrations when their spontane breathings are ready.Before the ınductions,1 minutes after the ınductions, during the extubation and 1,3,5,10 minutes after the extubation , Heart Rates,Sistolic ?Diastolic Arterial Pressures ,Mean Arterial Pressures are recorded.Results: The differences of SAB of Esmolol,Lidokain during the extubation and after 1,3,5,1O minutes extubation is significiantly higher according to deksmedetomin groups(p>0.05). After extubation, the differences of SAP of the groups of Lidokaine(1. and 3. minutes) is significiantly higher(p>0.05).At the same time, the SAP differences of Placebo,Esmolol and Lidocaine during the extubation and after the 5 and 10 minutes of extubation have no differences among the grops.İn Lidocaine Group The SAP differences of extubation, after the 1,3,10 minutes of extubation is significiantly higher according to beginnigs(p<0.05).Conclusion:We pointed that; Deksmedetomidinin is more useful for controlling to haemodynamic responses extubation when compared with the other groups.And also,we decided that we can use esmolol to control the haemodynamic responses succesfully.We have to pay attansion while using deksmedetomin,that?s why bradicardia is seen in 8 patients who have needed medication.
