Evaluation of hepatosteatosis and cardiovascular risk in patients with helicobacter pylori positive patients based on endoscopic diagnosis

OBJECTIVE: Helicobacter pylori infection is one of the most common bacterial infections in the world. It has been suggested that H. pylori may cause additional diseases outside the gastrointestinal tract secondary to the systemic inflammatory response caused by mediators released due to inflammation in the gastric mucosa. It has been suggested that H. pylori may have a role in cardiovascular and hepatobiliary diseases other than the gastrointestinal system. In this study, we aimed to evaluate the relationship of H. pylori with hepatosteatosis and cardiovascular risk using liver fibrosis scoring and cardiac risk scoring. MATERIAL-METHODS: In our study, 205 H. pylori positive patients with hepatosteatosis, 153 H. pylori positive patients without hepatosteatosis, and 110 H. pylori negative patients without hepatosteatosis were included retrospectively. The patients' laboratory findings (AST, ALT, GGT, ALP, LDH, T. Bilirubin, D. Bilirubin, serum albumin, lipid profile, hemogram), body mass index, H. Pylori biopsy and USG imaging results were recorded. According to the results, cardiac (Framingham, PAI) and liver fibrosis (FIB4, APRI, NFS) scores were calculated and the groups were compared. RESULTS: As a result of the statistical evaluation, BMI, weight and age were found to be significantly higher in the group with H. Pylori and NAFLD compared to the other groups (p<0.05). Glucose, AST, ALT, ALP, GGT, LDH, T. Cholesterol, Triglyceride, LDL, WBC, Hemoglobin elevation were found to be statistically significant in the group with H. Pylori and NAFLD compared to the other groups (p<0.05). PAI, Framingham, FIB4, APRI, NFS scores were found to be significantly higher in the group with H. Pylori and NAFLD compared to the other groups (p<0.05). CONCLUSION: In our study and the sample we took, it was thought that H. pylori was not a risk factor for either NAFLD or liver fibrosis, on the contrary, it was concluded that NAFLD was associated with cardiac risk. More comprehensive and new studies on this subject will reveal the true potential of this subject more clearly. Keywords: Helicobacter pylori, Hepatosteatosis, Cardiac Risk.
