Abstract (EN)
EVALUATION OF CLINICAL AND RADIOLOGICAL RESULTS OF SURGICALLY MANAGED ENCHONDROMAS Aim: In this study, the results of surgically treated pediatric and adult patients -who were diagnosed with Enchondromas- were compared. Furthermore, their clinical and radiological outcomes were evaluated. Material and Methods: In our clinic, among the patients who admitted to our out-patient Orthopedics and Traumatology clinics of Adnan Menderes University Hospital between October 2014 and October 2023, 56 patients were diagnosed with Enchondromas and were surgically operated. Those patients were operated by the same surgeon and retrospectively observed by sex, age, anatomic localization of the lesion, adjuvant substances usage, postoperative Consolidation, postoperative Complication and Tumor recurences. The results of surgically managed patients were then radiologically and clinically evaluated. Takigawa Enchondroma Classification was used in Hand-Phalangeal Enchondroma Patients. Results: In our study, 28 of 39 patients with the Hand Enchondroma were female and 11 of them were male patients. Among the 17 patients of the Long Bone Enchondroma, 14 were female and 3 were male patients. The average age was found to be 34.6 years in Hand Enchondroma patients and 48.1 years in Long Bone Enchondroma patients. According to anatomical localization, 33.3% of Hand Enchondromas were detected in the 5th Finger and 58.8% of Long bone Enchondromas were detected in the Femur. Hand Enchondromas were observed in 46.5% at the Proximal Phalanx region. Phalangeal lesions were evaluated according to the Takigawa Classification, 35.9% of the patients had Giant-Type and 64.1% had non-Giant hand enchondroma. There was not any significant difference detected among Adjuvant substances. Total postoperative consolidation at the 12th month was detected in 94.4% of patient, in whom Bone graft was used. Postoperative complication rate was high in Giant-type Enchondromas Postop Complication was evaluated according to whether Bone Cavity was left empty or was Filled with Bone Graft/Cement. No statistically significant correlation was detected. Tumor Recurrence was evaluated according to whether the Bone Cavity was left empty or filled with Bone Graft/Cement. Tumor Recurrence rate was found to be high in those with an empty bone cavity and was found to be low in those with Bone graft. Conclusions: Various procedures are performed in Enchondroma Surgery. Postoperative Clinical and Radiological results of the substances used as adjuvant treatment are similar. Postoperative consolidation of the cement material used as an adjuvant substance and also in the process of filling the bone cavity could not be evaluated. Tumor Recurrence is very low in cases where Bone Graft is used. The tumor recurrence rate of the bone cavity left empty is quite high..
