The effect of recurrent chemotherapy regimens on platin sensitive and resistant patients in epithelial ovarian cancer to survival time

Abstract (EN)
Objective: Ovarian cancer is one of the most common gynecological malignancies causing death all over the world and most common type of ovarian cancer is epithelial ovarian cancer. Majority of patients are diagnosed in advanced stages. Primary treatment approach is surgery in epithelial ovarian cancer. Then given adjuvant chemotherapy. In the study, paclitaxel and platinum combination, gemcitabine and topotecan given to platin sensitive and resistant patients which regime is more effective on survival time and tried to explain the side effect profile. Material and Method: In our study, patients has been operated between 05 January 2005 and 20 May 2014 and were included patients with a diagnosis of ovarian cancer in Cukurova University Department of Obstetrics and Gynecology. Research at Cukurova University Hospital Balcalı been obtained information from the archives of 92 patients were analyzed retrospectively. The diagnosis made surgery all patients were confirmed. According to telapsed time, platin sensitive patients received paclitaxel and platinum combination but platin resistant patients received one of the trio doxorubicine, gemsitabine or topotecan. Relapsed again when patients were given one of the trio has continued to repeat this regime. Socio-demographic characteristics, the time between relapses and hematological side effect profile were investigated on the study. Results: Looking at the socio-demographic data, survival rates were followed longer on patients who are under 45 years of age, the stage 3b and under, and multipar. Platinum and paclitaxel combination extends next recurrence time 11 months, gemcitabine 15 months, doxorubicin 12 months in given second line chemotherapy regimens. In third line regimens gemcitabine extends the next recurrence time 17 months, doxorubicine 21 months, topotecan 23 months. In total, %45 of patients was seen leukopenia, %44 eritropenia and %42 trombocytopenia. Conclusion: In terms of impact on survival between given the chemotherapy regimens is not considered a significant difference compared to each other when we look at the results. However, in patients receiving doxorubicin hematologic side effect profile it was observed less. The patients who receiving paclitaxel and platinum combination tolerability was better than any other regimens. In patients receiving neoadjuvant chemotherapy were occured less survival time because of selection increased comorbidity and with high tumor volume patients.
