Investigation of the effect of FHS/LH ratio in the early follicular phase on number of mature oocytes and embriyo development

Abstract (EN)
Before IVF procedure many tests including basal level of follicle stimulating hormone (FSH), estradiol (E2), inhibin B, ratio of follicle stimulating hormone to luteinizing hormone (FSH/LH)and antral follicle count are used. The purpose of our study is to determine the impact of the basal levels of FSH/LH ratio on success of IVF results. 648 patients were included in the study who underwent IVF treatment between 2012 and 2014. Patients were analysed by grouping into two according to their basal FSH/LH ratio. In group 1 (G1) FSH/LH ratio was smaller than 2 and constituted 473 patients and in group 2 (G2) FSH/LH ratio was bigger than 2 and constituted 175 patients. From the patient records day three hormon profile, antral follicle counti indication of in vitro fertilisation, ages of the partners, number of previous IVF cycles, IVF protocol, type and amount of the gonadotrophin used, total oocyte count (TOC), mature oocyte count (MOC), two pronucleus embryo score, transferred embryo count, result of pregnancy and E2 and progesteron levels on the day of hCG were obtained and transferred to computer by SPSS 21 programme. When the two groups were compared according to demographic properties it is found that in G2 FSH level on day 3 were higher in statistically significant amount, antral follicle counts were lower and ages of male and female were higher. In G2 reduced ovarian reserve and endometriosis were found to be higher whereas male factor and tubal factor were more abundant as etiology in G1 which was high in statistically significant amount. Dose of the used gonadotrophin were higher in G2 but TOC and MOC were higher in G1. Pregnancy and live birth rate were similar in both groups. We believe that FSH/LH ratio can be a reasonable parameter in showing the reduced ovarian reserve. We observed that the elevation of the level of FSH/LH ratio above 2 doesn't effect the rate of pregnancy and live birth rate negatively.
