Clinical significance of procalcitonin in cervico-vaginal secretions of women with preterm rupture of membranes

Abstract (EN)
Premature rupture of membrans (PROM), seen in 5-10 % of all pregnancies. PROM, is the most and unique diagnose that causes premature birth and intensive care unit. Also, the infection in mother and fetus has been found to be related with pulmonary hypoplasia ,umblical cord prolapsed, fetal anomalies and postnatal endometriosis. Because PPROM is related with preterm birth with 20-50%, was also related from the neonatal outcomes of prematurity.We aimed to assess the clinical value of procalcitonin level in servicovaginal secretions, which is a new inflammatory marker, in PPROM, at this study.This study was performed in Dicle University School of Mediçine, Department of Obstetrics and Gynecology, between September 2008 to September 2009, on 100 pregnant women. The approval of the cases has been taken about the study. The cases devided into two groups as patient group (n:50) and control group (n:50). Patients group were between 24-36 weeks of pregnancy and these women from traditional methods laid PPROM diagnosis, control group of pregnancies among 20-37 gestational week which was composed of pregnant women who were admitted for routine control. Hemogram, ultrasonographic evalution, urine examination, CRP and servicovaginal PCT levels were studied.In the patient group, CRP 25.42 mg / dl PCT 1.17 ng / dl and in the control group, CRP 6.22 mg / dl PCT level of 0.05 ng / dL were detected. Significant differences between these values were determined. Clinical cases detected amnionit CRP 56.10 mg / dl and PCT 2.85 ng/dL were detected. These values significantly higher than the clinical group was amnionitis not detected.Procalcitonin is new parameter which was used in the diagnosis of systemic or genarilize infection. Therefore there is no much information about the situation of PCT in pregnant women. Considering the etiology of infectious in PPROM when suspected for intrauterine infection in PPROM cases servicovaginal PCT levels were found highly.İn conclusion, servicovaginal secretion PCT levels can be used in fallow up of conservative manegment at PPROM for growth intrauterine infection risks.
