Objectives: Clinical course and immunological features and pathogenesis of Multiple Sclerosis (MS) is a quite heterogeneous illness; moreover, it is the most frequent cause of neurologic disorder at young and middle age groups. Even though its aetiology is not understood clearly, immune system has a critical role at RRMS pathogenesis. That some adipocytokines like leptin, resistin and adiponectin are correlated with inflammation and have also critical role at RRMS pathogenesis is one of the attention grabbing research subjects (1-6). In this study, our aim is to compare obtained datas between groups and to get data about immunologic mechanisms which affects MS pathogenesis transforming from KIS to RRMS, by obtaining relapsing remitting multiple sclerosis (RRMS), clinical isolated syndrome (KIS) facts and healthy volunteer's peripheral mononuclear blood cells and by controlling levels of serum leptin, resistin, adiponectin, monosit chemoattractant protein-1 (MCP-1), interleukin-1 (IL-1), interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor-1 (TNF-a). Moreover with this study results we aimed to reveal data that lead to new treatment methods with help of disability development indicators and prognosis that could be used at definitive diagnosis of RRMS patients. Material and Methods: At this study, we used 125 people who are followed at Dicle University Medical Faculty Education and Research Hospital. Fifty five of these patients have definite clinical MS diagnosis according to 2010 McDonald Criteria and are in remission, 30 of these have KIS diagnosis and 40 are healthy volunteers. We got once blood sample from patients and healthy control group. We compared levels of adiponectin, leptin, resistin, MCP-1, TNF-a, IL-1b, IL-6, IL-8 of patients and healthy control group. We compared studied parameters with age, sex,age at onset of clinical findings, clinical findings at first attack, disabilirty scores(EDSS) got during drawing blood sample and body mass index values. Results: It is determined that adiponectin, leptin, resistin, MCP-1 and IL-8 levels of RRMS cases are quite high (p<0,05). At comparements done with Dunn test, it is determined that levels of serum adiponectin, leptin, resistin and MCP-1 at RRMS cases are quite high at KIS group (at all comparements p<0,05). At the comparemednts between RRMS and healthy groups significant differences are determined for adiponectin, resistin, MCP-1 and IL-8 values (leptin and adiponectin- (p<0,05; resistin, MCP-1 and IL-8, (p<0,01). At the comparison between cases with optic neuritis (ON) and RRMS patients without optic neuritis, adiponectin, leptin, resistin and MCP-1 levels are quiet low at RRMS cases with optic neuritis (p<0,05). EDDS results of RRMS cases with optic neuritis are also lower (p=0,043). Conclusion: Results of this study showed that levels of some adiponectins increase during transformation from KIS to RRMS. It is possible that the increase at adiponectins which have proinflammatuar features lead to transformation from KIS to RRMS or at least it may have a role during this transformation process.
