Comparison and assesment of patients with intertrochanteric femur fracture treated with proximal femoral nail and hemiartroplasty

Objective: We are aimed to evaluate the proximal femoral nail (PFN) and hemiathroplasty that used in treatment of advanced age patient of intertrochanteric fractures that effect of on patient quality life.Materials and Methods: In this study, 113 patients who performed proximal femoral nail and hemiarthroplasty surgery due to intertrochanteric femur fractures were included between January 2006 and March 2011 period in Department of Orthopedic Surgery and Traumatology in Medicine of Gaziantep University. 43 patients who performed PFN and 70 patients who performed hemiarthroplasty surgery were selected group I and group II, respectively. Mean follow up was 12 months. According to operation time, amount of bleeding, early and late complications, reoperation ratio, duration of hospitalization and Modified Harris Hip Score were compared in each of two groups.Results: In PFN group, operation time, amount of bleeding, duration of hospitalization, reoperation ratio are lower than hemiarthroplasty group (p<0.05).In PFN group,late period seconder varus in hemiartroplasty group early and late period infections, late osteolizis and aseptic; loosing and use of the late osteolizis is the most important complications. According to Modified Harris Hip Score, excellent and good results especially high in PFN group. Both of two methods in use,patient age, kind of fracture and patient general situation important factors at which methods is more better about patient life quality and functional results. In PFN group have better results betwen 60-80 ages, in hemiarthroplasty group have better results over 80 ages.Conclusion: At stable intertrochanteric femur fracture, PFN surgeries which are performed with anatomic reduction and rigid fixation postop clinic and functional results are better than hemiarthroplasty surgery. Besides this surgery method (PFN) have more less revision ratio, amont of bleeding,operation duration, hospitalization and mobilization period than hemiarthoplasty surgery.Key Words: Intertrochanteric femur fracture, PFN, hemiarthroplasty.
