Abstract (EN)

Transportation has a magnificent prominence that is because modern human life strongly needs safe, secure, environmental, and sustainable transportation systems. Maritime transportation has a privileged position among the other transportation systems regarding its contribution to the global world trade. According to the UNCTAD' review of the recent maritime transportation report, over the 80 % of the world cargoes carried by shipping industry. On the other hand, maritime accidents have a tremendous potential for leading to catastrophic damage to crews, passengers, cargo, ship, and marine environment. Especially, fire/explosion accidents are one of the most encountered maritime accidents. At this point, International Maritime Organization (IMO) implemented some amendments on the ISM Code to improve maritime safety via the shift the corrective action paradigm to proactive approach in 2010. In this way, maritime shareholders might prevent accidents before they happened. However, this new paradigm also brings some challenges for the maritime shareholders such as; development of the advanced methods and the lack of research gaps in maritime literature based on the proactive approach. From this viewpoint, the research combines the HFACS and FCM methodologies in order to overcome the mentioned challenges. Firstly, PSC investigation reports and related competent authorities' reports are investigated in detail for the identified the cause of the fire related deficiencies. Next, these causes are classified accorging to the HFACS framework. After this point, FCM is utilized to prioritization the causes. Moreover, hiden root causes might have a great influence on systems, therefore; indirect relation between the deficiencies are also analysed. According to the revealed rood causes, Safe Ship System Mechanism (SSSM), Safe Ship Operation Mechanism (SSOM), Safe Ship Execution Mechanism (SSEM) is suggested in order to increase the effectiveness of the measures. Consequently, the study contributes to boht maritime industry and maritime safety literature. Developed the proactive model that reveals the root causes; therefore improve the fire-fighting capability is the industrial contiburion. The maritime safety literature contribution is that using advanced scientific methodologies, so close research gaps in the literature.
