Hemogram parameters in young cva patients and evaluation of thrombophilia panel

Abstract (EN)

Stroke is a disease with increasing socioeconomic importance in populations. According to the WHO, stroke is worldwide the second leading cause of mortality but ranks third in the developing countries. Stroke can be seen also in young adults aged between 15 and 55 years and called as young CVA. Young CVA causes serious loss of labor and production. The present study evaluated hemogram parameters and thrombophilia panel. Presence of inflammation during stroke was investigated and hemogram parameters, which are the indicator of this, were evaluated. Thrombophilia panel (Factor V Leiden, Factor II, MTHFR C677T and MTHFR A1298C mutations), which is among genetic factors -one of the unchangeable risk factors for stroke, and hemogram parameters before and after stroke, were compared between normal healthy subjects and young CVA patients and the difference was investigated. It was aimed to determine whether there is a relation between the parameters of thrombophilia panel and the parameters of hemogram as well as whether hemogram parameters and thrombophilia parameters would contribute to early diagnosis before stroke and to the diagnosis during stroke. For this purpose, 47 patients that met the definition of young CVA and had had stroke due to non-cardiac reason and 47 normal healthy subjects, who had not have stroke, were not obese, and had no hematological or rheumatologic disease, were included in the study. Hemogram parameters and thrombophilia panel were retrospectively reviewed. Exclusion criteria for the control group were being obese or having infectious diseases as well as inflammatory diseases such as FMF. As timely diagnosis is necessary for adequate treatment and for the prevention of recurrent strokes and serious sequela, knowing the risk factors of stroke and easily analyzed biochemical markers are needed. Inflammatory processes have important role both as the causes of ischemic CVA and in the pathophysiology of cerebral ischemia. In the present study, significant results supporting inflammation were obtained among hemogram parameters of young CVA patients during the first stroke. The present study demonstrated that, among hemogram parameters Hb, Htc, WBC, Neu/Lymphocyte ratio, RDW and PDW can be used as inflammatory markers and maybe even for risk assessment in stroke patients. It was reported that systemic inflammation plays an important role in the development of atherosclerosis and enhances the risk for stroke and coronary heart disease.
