Abstract (EN)
Purpose: In this study, serum visfatin levels of adult and adolescent gigantomastia patients were compared with serum visfatin levels in patients who had regular breast size or operated for other reasons without a diagnosis of gigantomasti. The aim of this study is to shed light on the etiopathogenesis of gigantomastia by considering the effect of visfatin on breast hypertrophy. Materials and Methods: The study was conducted in 2019 and included 31 volunteer patients who were diagnosed with bilateral gigantomastia or juvenile breast hypertrophy whose BMI 20 kg / m² or above (Group-1/study group) and 7 volunteers with BMI 20 kg / m² or above ( Group-2/control group) who had regular breast size or were not diagnosed with gigantomastia. The blood samples taken for the preoperative preparation from the patients were centrifuged in EDTA tube for visfatin level measurement and separated as serum and stored at -80º C. Results: According to Mann Whitney U test results, serum visfatin levels (Median = 10.38) of juvenile gigantomastia patients were significantly higher than control visfatin levels (Median = 5.53) (U = 42.00, p = 0.011). According to Mann Whitney U test results, no significant difference was found between body mass index (Median = 26,62) of juvenile gigantomastia patients and control group body mass index (Median = 24,17) (U = 77.50, p = 0.249). According to Mann Whitney U test results, there was no significant difference between the ages of the juvenile gigantomastia patients (Median = 29) and the control group (Median = 30) (U = 77.00, p = 0.438). Pearson correlation analysis showed no significant correlation between serum visfatin values and body mass index (r = -0.212, 38, p = 0.2). However, the relationship between serum visfatin values and body mass index is inversely proportional. Conclusion: High serum visfatin levels may be involved in the etiopathogenesis of breast hypertrophy. However, visfatin can be secreted from different areas such as visceral adipose tissue; additional studies are needed to determine the contribution of serum visfatin levels for diagnosis and treatment of gigantomastia or visfatin related endocrinological pathologies. In addition, serum visfatin values are affected by processes such as cancer, hormonal parameters and chronic inflammation; so that new controlled studies are needed to rule out patients menstrual cycles, subclinical infections or undiagnosed cancer cases.
