The effect of hormone levels on pre and postoperative morbidity and mortality in thoracic surgery

The relation between complications, mortality and hormone levels that is known as stress hormones such as ACTH, TSH, growth hormone, prolactin, cortisol, free and total T3-T4 were investigated. Also the changes in groups and relation between these subgroups were investigated. One hundred patients (59 male, 41 female) who underwent elective thoracotomy and were in ASA 1-lll, 18-80 ages were included.Blood samples for hormone levels were collected from patients before surgery, in postoperative 1st hour and postoperative 15th day. Samples were centrifuged and protected in -80 degrees Celsius. Patients were classified according to their diagnosis, operation and morbidity-mortality. The effect of all these parameters on morbidity and mortality was investigated.We found that there was no significance difference between tissue lose and tissue protected groups. Also we revealed that there was no difference between cancer and non-cancer groups. However, we established the high levels of cortisole and growth hormones in group has complications. There was significance decrease in levels of total T3 and T4 in complications group when complication and non-complication groups were compared to each other. We assigned that free T3, T4, cortisole and prolactin levels were significantly different when alive and dead patients were compared.We can not conclude whether changes of hormone levels depend on death or cause death, even we can state a significance relation between levels and complications or death. Whatever means on conclusion, changes in hormone levels and these were associated with postoperative condition of patients provides better and detailed evaluation of patients. Perhaps, with more detailed and large studies, hormone replacement or suppressive therapy will become a current issue.
