Evaluation of the relationship between complications after impacted mandibular third molar extraction and oxidative stress

Althought it is time to eruption, teeth that are not in the mouth for various reasons are called impacted teeth. Among all teeth, the mandibular third molar teeth are the most impacted teeth. Therefore, the impacted third molar operations are the most common procedures in maxillofacial surgery practise. Although various complications may occur at the end of these operations, the most common are pain, edema and trismus. These complications are known to occur as a result of an inflammatory response due to surgical trauma. Reactive oxygen species (ROS) have been shown in recent scientific studies to be related to many physiological and pathological conditions and take an active role in inflammation. However, although there have been many studies on oxidative stress in dentistry, no studies evaluating the relation between oxidative stress and complications occurring after the impacted mandibular third molar extraction have been found. Therefore, this study was performed to evaluate the relationship of impacted mandibular third molar to determine the level of oxidative stress in patients with teeth and postoperative complications. 47 patients with impacted mandibular 3rd molar teeth were included in the study. Saliva samples were taken before and 3 day after surgery and total antioxidant level (TAS), total oxidant level (TOS) and oxidative stress index (OSI) values were examined. Also, preoperative and postoperative 3rd day edema, pain and mouth opening values were recorded. As a result of the evaluations, it was determined that there was a significant edema, trismus and pain scores on the 3rd postoperative day, and TAS, TOS and OSI values decreased significantly on the 3rd postoperative day (p<0,001). However, there was no correlation between postoperative TAS, TOS and OSI values and postoperative complications, but there was a positive correlation between preoperative TAS and edema, preoperative TOS and trismus. Based on these findings, it can be said that there is a relationship between surgical intervention and trauma and oxidative stress.
