Retrospective evaluation of Hashimoto thyroiditis patients in terms of serum atherogenicity index

Abstract (EN)
Aim: We aimed to evaluate the importance of serum atherogenicity index in terms of cardiovascular disease risk in patients with Hashimoto's thyroiditis. Material and method: Our study included 109 patients who had previously been diagnosed with Hashimoto's thyroiditis, who applied to the Internal Medicine outpatient clinic of Zonguldak Bülent Ecevit University Faculty of Medicine between 2011-2021. Thirty-two healthy individuals without a diagnosis of Hashimoto's thyroiditis or TSH level disorder, known as the control group, were randomly selected. Demographic datas, height, weight, body mass index, systolic and diastolic blood pressures, hemogram, TSH, fT3, fT4, Anti-TPO, Anti-Tg, erythrocyte sedimentation rate, CRP, total cholesterol, triglyceride, LDL -K, HDL-C datas of all individuals included in the study were analyzed. Serum atherogenicity index and Framingham risk score were calculated for all individuals. Results: The mean age for the patient group was 45.36±14.63 (years) and the mean age for the control group was 34.13±8.91 (years). The age (p=0.000) and gender (p=0.080) datas of the groups were similar. Autoimmune disease, diabetes mellitus and hypertension were found to be significantly more common in patients diagnosed with Hashimoto's thyroiditis (respectively; p= 0.039, p= 0.007, p= 0.001). While the Anti-TPO positivity of the patient group was 94.5%, the Anti-Tg positivity was 39.4%. When the mean lipid blood levels were examined, it was determined that the total cholesterol, LDL-C, and triglyceride levels of the patient group were significantly higher than the control group (respectively, p= 0.018, p= 0.044, p= 0.000). The mean serum atherogenicity index was calculated as 0.438±0.28 in the patient group and it was found to be higher in the patient group than in the control group (p=0.001). Framingham risk score was calculated for all individuals and Framingham risk score was found to be high in favor of the patient group (p=0.002). In addition, Framingham risk score and serum atherogenicity index were found to be correlated in both the patient group and the control group (in the patient group; r=0.449, p=0.036, in the control group; r=0.371, p=0.001). The mean body mass index of the patient group (29.20±7.25 kg/m²) was found to be statistically significantly higher (p=0.004). In addition, the relationship between body mass index and serum atherogenicity index was statistically significant in both the patient group (r=0.467 and p=0.000) and the control group (r=0.565, p=0.001). Conclusion: According to the results of our study, HT patients had a higher serum atherogenicity index and was associated with the Framingham risk score. We think that the serum atherogenicity index, such as FRS, can also be used in terms of cardiovascular diseases when monitoring HT patients.
