It?s a known fact that the safest and fastest traveling system around the world is air transportation. However, how safe it may be, still a lot of accidents occur in this high risk environment. After investigating the causes of these accidents, it has been realized that the human factor (pilots, air traffic controllers, maintenance units, designers, manufacturers, etc.) is the common link to all accidents, and within the `human? factor the flight crew holds the major proportion.Although there has been a great technologic progression in aviation, the progression in the personnel using this technology is lacking parallelism. Especially in using advanced systems like aircrafts, the role of individuals composing a team in order to succeed a safe flight, is of great importance. It is seen that, only via flight crew coordination -which is the proper and effective use of every possible source- correct decisions and safe flights are successfully executed.This study initially emphasizes on the human factor and its main element which is the flight crew in order realize the surrounding threats and finally the human performance and the factors affecting this performance from past to present. Aviation accidents and the human factors pertaining to these accidents has been investigated and elucidated. Flight crew factor was adapted to Human Factor Analysis and Classification System that is one of the human factors analysing models and it was suggested as a new model. It is foreseen that the accident ratio can be reduced by means of accurate and efficient determination of the flight crew factors contributing to accidents, using the gained new model.
