Examination of the association between antibodies against helicobacter pylori and various cliniccal situations

In the present study, we aimed to determine the association between gastric colonization of Helicobacter pylori, histological grading of gastritis and the duration of the H. pylori infection with dyspepsia and the presence of anti- H. pylori IgG, IgA and IgM. Ninety-seven dyspeptic patients were included in the study. Each patient underwent blood sampling and endoscopy. Four antral biopsy specimens were obtained from these patients. The severity of antral gastritis was graded by pathologists who were unaware of the serological and endoscopic findings. The presence of H. pylori in the biopsy specimens was determined by Gram staining, culture and urease tests. After two mounts the same procedure was repeated. H. pylori were positive in 86 (88.66%) patients with Gram staining and 93 (95.88%) patients with in-house ELISA for anti-//. pylori IgG. Using microscopy as a reference test the sensitivity, specifity, positive predictive value and negative predictive value of in-house ELISA for serum anti-//. pylori IgG were 97.67%, 18.18%, 90.32%o, 50%, respectively. Anti-//. pylori IgA was tested in 86 patients with dyspepsia. Seventy-one of these patients (82.55%) were positive with commercial ELISA for anti-//. pylori IgA. Using microscopy as reference test, the commercial ELISA for anti-//. pylori IgA was 83.33% sensitive, 25.00% specific with 91.54% positive predictive and 13.33%» negative predictive values. Eighty-eight patients were screened with a commercial ELISA for anti-//. pylori IgM. Only 5 patients (5.68%) were positive with this test. The serology of anti-//. pylori IgM had no diagnostic value.80 In thirty-one patients, serological characterisation was carried out using western blot analysis. In all subjects, anti-/£ pylori IgG was detected irrespective of H. pylori presence. Immunoreactive bands in 130 kDa moleculer weight (CagA) were found in 15 (48.38%), 87 kDa moleculer weight (VacA) were found in 25 (80.64%) of the patients. The major common immunoreactive components were at 94, 87, 84, 74, 58 and 54 kDa. From the serum samples of 31 patients in 8 (25.81%) 130 kDa, in 7 (22.58%) 35 kDa, in 6 (19.35%) 64 kDa, in 5 (16.13%) 61 kDa moleculer weight immunoreactive proteins were disappeared in the second serum samples.
