The effect of early noninvasive respiratory support for the patients with acute respiratory failure in the hematology clinic

Abstract (EN)

Purpose: The purpose of this study was to evaluate the effect of early noninvasive respiratory support on mortality in patients with acute respiratory failure in hematology clinic. Material and Method: Patients who developed acute respiratory failure during follow-up in hematology clinic were included in the study. On the day acute respiratory failure began, noninvasive respiratory support was given by the respiratory physiotherapist without admitting the patients to the intensive care unit (mode: BIPAP-ST). The parameters of acute respiratory failure of the patients were monitored daily. The patients who developed ICU indication were referred to the ICU. Findings: The study included 13 patients, of whom 5 were female and 8 were male (average age 63.0±.7 years). Mortality rate was found to be higher in cases with unsuccessful noninvasive respiratory support (p=0.02). Of the cases; When the expected mortality rates were evaluated according to APACHE II scores, higher mortality rates than expected were not observed. However, when successful and unsuccessful groups with noninvasive respiratory support were examined, the success rate was higher in autologous transplantation and in cases without bone marrow transplantation (autologous transplantation 83.33%, allogeneic transplantation 0%, bone marrow transplantation 75%). Conclusion: Acute respiratory failure is a cause of mortality in patients with hematologic malignancies. If early signs of respiratory failure are observed, the results of early noninvasive respiratory support have curative effects.
