Evaluation of the problems and affecting factors in diabetes management in people with diabetes WHO receiving and not receiving hemodialysis treatment

Abstract (EN)
This descriptive study aimed to determine the diabetes management problems experienced by people with diabetes who receive and do not receive hemodialysis treatment and the factors affecting these problems. The study was conducted with 160 diabetic patients, 80 of whom were receiving hemodialysis treatment. Data collection tools used in the study were Descriptive Form, Problem Areas in Diabetes (PAID), Patient Health Questionnaire (PHQ-9), and Gold Hypoglycemia Scoring. Data were analyzed with descriptive statistics, frequency and percentages, Spearman's correlation analysis, multiple linear regression analysis, Mann Whitney U test, Chi-Square and Fisher Exact test by using MedCalc Statistical Software v.12.7.7. Significance level was accepted as p<0.05. The PAID scores of patients who received and did not receive hemodialysis treatment did not have correlation. Patients who received hemodialysis treatment had more impaired glycemic control than patients who did not, significantly (p<0.05). The patients receiving hemodialysis treatment had more problems with weight control and regular exercising (p<0.05). There was no difference between the groups in terms of depression. In both groups, it was determined that the increase in depression significantly increased diabetes problems (p<0.05), and the younger patients with diabetes had more diabetes problems than the older patients. More frequent and regular blood glucose monitoring, specific care plan to improve glycemic control, assessment of special needs for patients with retinopathy and foot ulcer, support for weight control and regular exercise should be provided for patients with diabetes and receiving hemodialysis treatment. Regardless of hemodialysis treatment, in the presence of depression in all patients with diabetes, more intensive and specialized diabetes care should be provided.
