Long term results of treatment of Hirschsprung's disease

Abstract (EN)
Purpose: The purpose of the present study was to evaluate the patients who treated due to Hirschsprung?s disease in the last 20 years period.Materials and Methods: A total of 116 patients who operated because of Hirschsprung?s disease between 1988?2008 were evaluated file records and 50 patients who communicated were evaluated for long term results. The patients? type of surgical technique, presentation, intermarriage, operation age, associated anomalies, the level of aganglionic segment, gender and current age were evaluated.Results: The male/ female proportion of 50 patients who operated because of Hirschsprung?s disease in our clinic was 34/16.The level of the aganglionic segments were rektum in 5 (%10) patients, sigmoid colon in 39 (%78) patients, transvers kolon in 5 (%10) patients and descending colon in 1 (% 2). Intermarriage was present in 26 (%52) patients, associated anomalies were present in 10 (%20) patiens. There was parental history in one patient (%2). The time passed after definitive surgery was 4?23 years. The current age of the patients were between 4?25 years. Patients surgical technique were Duhamel Martin in 32 patients, Transanal Endorektal Pullthrough in 11 patient and SoaveBoley modification in 7 patients. The most seen complications of the patients evaluated for long term outcome were soiling in %38, distention in %34, constipation in %32, enterocolit in %18, growth retardation %12, urinary incontinence in %8 and encompresis in %8.Conclusions: The treatment of Hirschsprung?s disease is not only the surgical correction of the pathology, but it should include the treatment of surgical and physiological complications as well. After the surgical repairment the bowel functions getting beter in time. The surgical procedures aren?t the last step in Hirschsprung?s disease and it should be keep in mind that these patients? necessity of medical treatment continue until adult time.Keywords: Child, hirschsprung?s disease, pull-through, long term outcome and treatment
