The effect of uterine arterial doppler measurements, immune profile and high sensitive c-reactive protein levels on the pregnancy and fetal outcome in second trimester pregnants

This study was designed to predict pregnancy complications by determining maternal serum cytokine levels and uterine artery Doppler blood flow measurements at second trimester.This study was performed on 88 healthy pregnant women who received routine prenatal care. The demographic features were reported. In the second trimester, cytokine and high-sensitive C-reactive protein (hs-CRP) levels were obtained in venous blood sample from all participants and uterine artery Doppler ultrasonography was performed to measure blood flow and the relationship of pregnancy complications was determined.When complications and results were considered, it is found that the incidences of preeclampsia (PE), intrauterine growth restriction (IUGR), early membrane rupture (EMR) and preterm delivery (EDT) were noticed as 11.3 %, 10.2 %, 6.8 % and 4.5 %, respectively. It was determined that the levels of IL-4, IL-10,TGF-b1 and IFN were significantly higher in preeclamptic women when compared with normal pregnants. There was a significant relationship between presence of notch and occurance of preeclampsi. The sensitivity, spesificity, positive and negative predictive values of notch positivitiy to determine PE were 80 %, 82 %, 36 % and 96 %, respectively. No significant relationship between cytokine levels and pregnancy complications were found except preeclampsia.As a result, uterine arterial Doppler measurements were highly sensitive to predicting PE. There was no relationship between cytokin levels in second trimester and pregnancy complications except preeclampsia. The levels of IL-10, IL-4, TGF-ß1, IFN-? examined in 20-24 weeks of pregnancy were found to be important to predict PE and complications of PE.Key Words: Pregnancy, uterine arterial Doppler, cytokine, interleukin, C-reactive protein.
