The production of cytokines in patients with infected diabetic foot, relationship with acute decompensated heart failure and effects on prognosis

We know that cytokines have an imprtant roles in the development of congestive heart failure. In this study we planned to show the roles of cytokines with acute decompensated heart failure in patients with infected diabetic foot.In this study we analyzed 26 individuals (17 male, 9 female, average age of 61,88± 6,8) who applied to Çukurova University Medical Faculty Hospital diagnosed as acute decompensated heart failure and infected diabetic foot. We analyzed tumor necrosis factor alpha (TNF-?), interleukine-1ß (IL-1ß), interleukine-2 (IL-2), interleukine-6 (IL-6), interleukine-10 (IL-10) and echocardiographic measures (EKO) of patients when applied to hospital. After treatment of diabetic foot medically or surgery and improving congestive heart failure the measurements were repeated. Control group is constituted from diabetic patients who have not any infected diabetic foot and controlled well.The amount of TNF-?, IL-1ß, IL-2, IL-6 and IL-10 was increased in patients. We found a significant opposite correlation between left ventricule ejection fraction (EF) and amount of serum TNF-? (p<0,05), IL-1ß (p<0,05) ve IL-6 (p<0,01). In surviving patients we determined a significant relationship between increasing of left ventricule EF and decreasing amount of serum IL-6 (p<0,01). We observed an important increase in the amount of serum TNF-? and IL-6 in died patients compared to surviving patients (p<0,05 ve p<0,05).Results of this study suggested that TNF-?, IL-1ß and most importantly IL-6 may play role in decompensation of heart failure. We determined that TNF-? and IL-6 may be predictor of acute decompensation of heart failure and mortality. We thought that in acute decompensated heart failure, treatment strategies targeting IL-6 may be attempted.Key Words: acute decompensated heart failure, cytokines, infected diabetic foot
