Frequency of non-alcoholic fatty liver disease in inflammatory inflammatory diseases and its affecting risk factors

Abstract (EN)

In this study, it was aimed to determine the incidence of non-alcoholic fatty liver disease (NAFLD) in patients diagnosed with Inflammatory Bowel Disease (IBD) and to evaluate it in terms of possible risk factors. For this purpose; 207 patients followed up for Inflammatory Bowel Disease in Tokat Gaziosmanpaşa University Medical Faculty Hospital Gastroenterology Outpatient Clinic were retrospectively analyzed. Height, weight, platelet values, aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma glutamyl transferase (GGT), bilirubin, C-reactive protein (CRP), sedimentation rate, fasting blood sugar (FSU), lipid profiles, insulin levels, and presence of insulin resistance were determined on the follow up of the patients. The presence of NAFLD was evaluated with ultrasonography, computed tomography (CT) and magnetic resonance (MR). The degree of adiposity was determined ultrasonographically. In order to evaluate the increase in the frequency of NAFLD in IBD compared to the normal population, patients who applied to the outpatient clinic due to dyspepsia and whose liver imaging and other metabolic parameters were recorded were selected as the control group, and comparisons were made. A total of 120 people were included in our study, of which 66.7% (n=80) constitute the case group and 33.3% (n=40) constitute the control group. 50% (n=40) of the case group consists of ÜK and 50% (n=40) of CDs. The control group was selected from patients with functional dyspepsia. In terms of gender, 44 people (36.7%) included in the study were men and 76 people (63.3%) were women. 38.8% (n=31) of the case group were male, 61.3% (n=49) were female; 32.5% (n=13) of the control group were male and 67.5% (n=27) were female. The mean age of the people included in our study was 48.3±14.7 years. Although the incidence of diabetes, hypertension and hyperlipidemia was higher in the case group than in the control group, there was no significant difference between the groups (p values: 0.2; 0.8; 0.8, respectively). The frequency of non-alcoholic fatty liver disease was found to be 32.5% in both the case and control groups (p: 1). The total number of NAFLD patients in the entire study group was 39. 30.3% of them were female and 36.4% were male. In general, the frequency of NAFLD was higher in those with dyslipidemia. The frequency of NAFLD in the whole study group was 53.8% in diabetics, 27.3% in hypertension, and 40% in hyperlipidemia (p values: 0.11; 0.56; 0.72, respectively). The total number of NAFLDs among those with Inflammatory Bowel Disease was 26. In this group, NAFLD was not observed in those with a history of resection (p:0.007). Non-alcoholic fatty liver disease was observed in 63.6% of patients with diabetes in the Inflammatory Bowel Disease group (p:0.03). In conclusion, in our study, it was observed that disease-related factors as well as metabolic processes (DM, HL, high BMI) increased the incidence of NAFLD in IBD patients.
