Abstract (EN)

Compliance with treatment and family support are important for the successful maintenance of diabetes treatment. Non-compliance with treatment increases the rate of developing complications, mortality, admission to the emergency room, and hospitalization and decreases the quality of life of individuals. This study aimed to compare the perception of treatment, treatment compliance, family support/conflict status of patients who had newly started insulin therapy and who had been continuing their treatment for 2-5 years and determine the relationship between the scales (type 2 dm patient compliance, family support and family conflict, insulin therapy evaluation) and between acute complications and the scale scores. This descriptive study was carried out with 118 patients between March and November 2021. The sample was determined with G-power analysis. Data were collected using an Individual Information Form, the Scale for Treatment Compliance in Type 2 Diabetes Mellitus, the Diabetes Family Support and Conflict Scale, and the Insulin Therapy Assessment Scale. Kruskal Wallis H test and Mann Whitney U test were used in data evaluation. The mean age of the patients who had been using insulin for 0-1 years was 54.76±12.15 years; the mean duration of diabetes was 9.32±6.7; the levels of family support (10.00±3.96) and treatment compliance (81.12±11.36) were moderate; the mean total score for the perception of insulin therapy (41.75±8.16) was low; the family conflict score (11.19±2.25) was high; 42.4% of the patients had hypoglycemia 1-3 times; 27.1% had hyperglycemia. It was determined that marital status affected the perception of insulin therapy. A statistically significant difference was found between experiencing hypoglycemia and patient compliance (p<0.05). There was a significant positive correlation between the "Scale for Treatment Compliance in Type 2 Diabetes Mellitus" and "Family Support (r=0.376; p<0.05)", "Family Conflict (r=0.329; p<0.05)", and "Family Support" and "Family Conflict (r=0.342; p<0.05)". The mean age of patients who had been using insulin for 2-5 years was 55.64±12.03; the mean duration of diabetes was 11.39±5.65; the levels of family support (10.42±4.30) and treatment compliance (80.37±11.68) were moderate; the total insulin therapy score (43.00±7.71) was low; the family conflict score (11.54±3.13) was high; 54.2% of the patients had hypoglycemia 1-3 times; 23.7% had hyperglycemia 1-3 times. It was found that marital status, economic status, and family structure affected the level of patient compliance and that education level, economic status, and family structure affected the perception of insulin therapy. A statistically significant difference was found between experiencing hypoglycemia and the perception of insulin therapy and the level of family conflict (p<0.05). There was a significant difference in terms of experiencing hyperglycemia and the level of family support (p<0.05). The level of family support was lower in patients who had never experienced hyperglycemia compared to patients who had experienced hyperglycemia at least once. There was a significant positive correlation between the scores on the "Scale for Treatment Compliance in Type 2 Diabetes Mellitus" and "Family Conflict (r=0.412; p<0.05)" and "Family Support" and "Family Conflict (r=0.289; p<0.05)" and between the scores on "Family Conflict" and total "Insulin Therapy Assessment Scale (r=0.332; p<0.05)".
