Evaluation of Transdiaphragmatic Passage of İntraperitoneal İnjected Labelled E.Coli Through Fibrin Sealant (Vivostat®) Coated Diaphragm

Abstract (EN)

Transdiaphragmatic passage of bacteria and bacterial products is very important insystemic effects of intraperitoneal infections. In many studies it is shown that blockage oflymphatic stomata may decrease mortality but none of these methods could be clinicallyapplicated. Vivostat® is a bioadaptive, highly adhezive and elastic fibrin sealant that can beeasily and safely applicated to surgical surfaces with its rapid polimerization. There is nocertain information about the blocking effects of fibrin sealant on diaphragma. In this studywe aimed to show the coating effects of vivostat on diaphragmatic lymphatic stomatas andbacterial migration through stomata.Sixteen (2 rats for preperation) adult Wistar- albino rats of 280-300 grams were dividedin 2 groups. Tc 99m pertecnetat labeled E coli is applied to infradiaphragmatic area withlaparotomy in Group 1 (n=7). In group 2 (n=7) the same process was performed after thediaphragma was coated by Vivostat®. Sintigrafic dinamic images were captured at the 20thminute after the abdomen was closed and static images were captured at 1st, 2nd and 6thhours. Rats were sacrified after the 6th hour static screening. Intracardiac blood samples weretaken for blood cultures and radioactivity counts and hepatectomy and spleenectomy wereperformed for tissue radioactivity counts.Early diaphragmatic passage to mediastinal lymphatic system and systemic bloodcirculation were observed in dinamic sintigraphic images for both groups. All the bloodcultures were positive. In group 1 and 2 radioactivity count result were respectively3,4x105±1,2x105, 3,9x105±1,5x105 cpm/ml/min (p=0,655), 5,7x104±2,5x104 7,7x104±2,4x104cpm/gr/min (p=0,277), 3,2x104±1,6x104, 3,5x104±1,8x104 cpm/gr/min (p=0,749) in blood,4liver and spleen samples and there was no statisticaly significant difference between twogroups.This study shows that fibrin sealant (Vivostat®) does not block transdiaphragmaticpassage of labeled E Coli.Key Words: Intraperitoneal, labeled E Coli, vivostat, transdiphraghmatic passage
