Root cause of work accidents

In this study, the occupational accident is explained according to national and international legislation. Occupational accident statistics in Turkey were examined. Occupational accidents in different sectors were examined and root causes were found according to their occurrence. When examining work accidents, accidents occurring in Turkey were taken into consideration. The data collected through the examined work accident reports, eyewitness statements, Social Security Institution work accident notifications, law enforcement reports and crime scene investigations. The application principle of the FMEA method used in root cause analysis is explained. In the light of the collected data, the root causes were found by using the failure mod and effect analysis, which are among the risk assessment methods, and at the same time they were expressed numerically. In the root cause analysis, improvement was proposed for each root cause and after these recommendations, the root cause risk was reviewed again.
